आज मैं मेरा व्यक्तिगत जीवन और परिचय आप सबके साथ साझा कर रहा हूं। प्रकाश S/o गोबरराम जी (डोंगी) चौधरी संस्थापक:- श्री राजेश्वर हेल्थ फाऊंडेशन। 
आप सबकी जानकारी के लिए मेरा व्यक्तिगत परिचय दे रहा हूं। मेरा जन्म 26/1/1995 एक गरीब  किसान परिवार में हुआ, मेरी शिक्षा ननिहाल गांव कनाना में सन 2007 में 7वीं पास की, सन 2007 मे मेरे मामा जी श्री नेमाराम जी (धूणिया) मुझे पढ़ने के लिए बल्लारी कर्नाटक ले गए और उसी वर्ष मेरे पिताजी का स्वर्गवास हो गया, उसे समय मेरी आयु 12 वर्ष थी। मेरी शिक्षा 8th पास इंग्लिश मीडियम स्कूल बल्लारी कर्नाटक RSS स्कूल में हुई, सन 2009 में मेरे मामा जी ने मुझे आर्थिक मदद करके व्यवसाय कराया,  उस समय मेरी आयु (14) वर्ष थी तभी मैने खुद के व्यवसाय की शुरुआत की 2009 से 2011 तक  कोप्पल कर्नाटक में की और 2011 से 2015 तक गंगावती कर्नाटक उसके बाद मेने 2015 में अहमदाबाद गुजरात में गारमेंट मैन्युफैक्चरिंग का व्यवसाय शुरू किया। इस व्यवसाय से मैंने मेरे जीवन के हर एक सपने को पूरा किया। सन 2015 में मैने कार खरीदी और मेरा सपना था अहमदाबाद में खुद का मकान हो, यह मेरा सपना 2019 सफल हुआ, ईश्वर के आशीर्वाद से आज अहमदाबाद में मेरे दो फ्लैट है, मैं एक सफल बिजनेसमैन बन गया था। सन 2020 में कोरोना काल के समय मेंने लॉकडाउन में मेरे पास कार्य कर रहे 28 लोगों के खाने पीने, रुकने एवं प्रत्येक व्यक्ति को ₹5000 आर्थिक सहायता देखकर गांव पहुंचाया। उसके बाद मैं मेरे परिवार के साथ मेरे गांव पारलू जिला बालोतरा राजस्थान आया। गांव आकर (2020) मे मैने मकान निर्माण कार्य शुरू करवाया, जिसका निर्माण कार्य पूर्ण रूप से सन 2024 में पूर्ण हुआ इस बीच मैंने 27 बीघा कृषि भूमि खरीदी, और गृह प्रवेश की तैयारीयों करने लगा, गृह प्रवेश के 1महीने पहले मेरी माता जी को कैंसर बीमारी हो गई, जिसके कारण मुझे कई दुखों का सामना करना पड़ा। मैं पूरी तरह टूट गया था, माता जी के लिए क्या करूं और गृह प्रवेश के लिए क्या करूं, माता जी की कैंसर बीमारी के लिए मेरे परिवारजन और मुझे व्यक्तिगत जानने वाले ने मेरी पूरी सहायता की, माताजी के लिए उत्तम हॉस्पिटल व्यवस्था कैंसर बीमारी के बारे में प्रत्येक प्रयास किया जिससे मेरी माता जी कैंसर मुक्त हो, फिर मैंने गृह प्रवेश के बारे में माताजी से बातचीत की, माताजी ने इच्छा जाहिर की गृह प्रवेश किया जाए और कुलदेवी को लाया जाए, मेरी माताजी एवं परिवार जनों मार्गदर्शन से मेरी इच्छा के अनुरूप मेरी कुलदेवी माता नागणेची जी को पूरे विधि विधान के पूरे परिवार जनों के साथ जाकर साथ लाया,और जागरण एवं महाप्रसादी कार्यक्रम बड़े धूमधाम से 5 म‌ई 2024को कार्यक्रम संपन्न हुआ, मेरे इस  घर में पूरी सुख सुविधाएं है मेरी इच्छा के अनुकूल है। मेरे माता जी का इलाज हाल में एम्स हॉस्पिटल जोधपुर में चल रहा है, माताजी के कैंसर इलाज के दौरान मेरी जो हॉस्पिटल यात्रा और जो मुझे अनुभव मिला वह कई कैंसर पीड़ित लोगों के काम आएगा, इसी दौरान मुझे सोशल मीडिया के माध्यम से जानकारी मिली, कैंसर पीड़ितों के लिए 85 वर्ष बुजुर्ग व्यक्ति द्वारा झांड़ा लगाया जाता है, फिर मेरी भी इच्छा हुई मैं भी मेरे माता जी के साथ वहां जाऊं, उस गांव का नाम देवमाली गांव है जिला ब्यावर है, मैं और मेरी माता जी और मेरे प्रिय मित्र बाबूराम जी के साथ देवमाली गांव गया, और वहां की तमाम जानकारी ग्रामीण एवं पीड़ितों के माध्यम से प्राप्त की, वहां की व्यवस्था और वहां के लोग अति प्रिय लोग हैं, वहां पर में तीन रात रुका और देव दर्शन किये, मैं वहां पर चल रही व्यवस्था से प्रभावित हो गया, वहां पर स्थित भगवान श्री देवनारायणजी एवं भेरुजी महाराज मंदिर है, वहां के लोग सतयुग वादी लोग हैं पूरी तरह ईश्वर पर विश्वास रखते हैं और अपना जीवन व्यापित करते हैं। वहां पर जो झाड़ा लगते हैं, उनकी आयु करीबन 85 वर्ष है और सुबह 7:30 से शाम 6:00 बजे तक झाड़ा लगते हैं, और प्रत्येक दिन तीन-चार हजार कैंसर पीड़ितों का झाड़ा लगाया जाता है। संपूर्ण भारतवर्ष से कैंसर पीड़ित पहुंच रहे थे। वहां पर मैंने कई कैंसर पीड़ितों को देखा और मेरे मन में भी एक प्रेरणा आई कि मैं भी निःशुल्क कैंसर पीड़ितों की मदद कर सकता हूं, मैने सोशल मीडिया के माध्यम से 26 जनवरी 2025 को मेरे जन्मदिन के अवसर पर मैंने प्रण लिया, आजीवन कैंसर पीड़ितों के लिए निःशुल्क आर्थिक सहायता करूंगा, मेरा उद्देश्य निःशुल्क कैंसर पीड़ितों का इलाज करवाना, मैंने सोशल मीडिया पर एक मुहिम चलाई जिसका नाम कैंसर मुक्त भारत अभियान रखा, आप सब कि जानकारी के लिए बता रहा हूं मेरे सोशल मीडिया सभी प्लेटफार्मों को मिला कर 38000 हजार फॉलोअर हैं। सोशल मीडिया का सदुपयोग करते हुए मैंने कैंसर मुक्त भारत हो इसके लिए काम करना शुरू किया, कैंसर मुक्त भारत अभियान मुहिम के तहत कुल 26 लोगों ने मेरे से संपर्क किया, मैंने उनको हर संभव मदद का भरोसा दिया, फिर मैंने सोचा सभी समाज के संपन्न लोगों के साथ मिलकर  एक राष्ट्रीय स्तर पर फाउंडेशन बनाया जाए और निःशुल्क आर्थिक सहायता दी जाए, और आर्थिक रूप से गरीब तमाम लोगों की निशुल्क आर्थिक सहायता की जाए। जिसके लिए मैंने (26 जनवरी 2025) को श्री राजेश्वर हेल्थ फाउंडेशन की स्थापना कर दी, इस फाउंडेशन  का मुख्य उद्देश्य निस्वार्थ कैंसर पीड़ितों मदद करना, इस फाउंडेशन के माध्यम से कहीं गांवों में निशुल्क एंबुलेंस की व्यवस्था करेंगे। मेरे प्रिय जनों कैंसर एक ऐसी बीमारी है जिससे पीड़ित की मौत अति दुःखों से होती है और परिवार को आर्थिक रूप से पूरी तरह बर्बाद कर देती है। ऐसे मैंने मेरे जीवन में कई उदाहरण देखे हैं जिसकी जानकारी मेरे पास है, इस बीमारी का इलाज प्रत्येक व्यक्ति के संभव नहीं है, इस बीमारी के कारण मैंने मेरे कई रिश्तेदारों को खोया है, साथियों मेरी आर्थिक स्थिति ठीक है, और मैं एक सक्सेस बिजनेसमैन हूं, मैंने बिजनेस में वह सब सफलता हासिल की जो हर बिजनेसमैन चाहता है लेकिन इस बीमारी के कारण मैं पिछले 1 वर्ष से व्यवसाय के लिए अहमदाबाद नहीं जा पाया, लेकिन मेरे आराध्य देव श्री राजेश्वर भगवान की असीम कृपा से मैं पूरी तरह संपन्न हूं, राजेश्वर भगवान की असीम कृपा से मेरे परिवारजन एवं रिश्तेदार सभी आर्थिक रूप से संपन्न है। कैंसर मुक्त भारत अभियान मुहिम की शुरुआत देवमाली से की और मेरा पूरा जीवन कैंसर पीड़ितों के लिए समर्पित किया। मेरा जन्म एक ऐसी विशाल समाज में हुआ है, मेरी समाज का नाम आंजणा पटेल कलबी समाज। मेरी समाज के आराध्य देव श्री राजेश्वर भगवान ने 100 वर्ष पूर्व में कई भविष्यवाणियां की थी, वह सत्य हुई, मेरी समाज ने देश मे अनेकों समाज सेवा के कार्य किया ,मेरी समाज के लोग मुख्य रूप से खेती-बाड़ी और संपूर्ण भारतवर्ष में व्यवसाय करते हैं। मेरे समाज की संपूर्ण भारत में अनेको संस्थाएं समाज सेवा का काम कर रही है। मेरी समाज के मुख्य तीर्थ स्थल श्री 1008 श्री राजारामजी आश्रम (  गादीपति महन्त पूज्य श्री 1008 दयाराम जी महाराज) शिकारपुरा लूणी तहसील में स्थित है। श्री  Lord Shri Rajeshwar Bhagwan, the deity of my community.
Today, I am sharing my personal life and introduction with you all. I am Prakash S/o Gobarram Ji (Dongi) Choudhary, the founder of Shri Rajeshwar Health Foundation.
For your information, I am providing my personal introduction. I was born on January 26, 1993, into a poor family. I completed my 7th grade education in 2007 in my maternal grandparents' village, Kanana. In 2007, my maternal uncle took me to Ballari, Karnataka, for education, and in the same year, my father passed away when I was 14 years old. I completed my 8th grade education in an English medium school in Ballari, Karnataka, run by RSS. In 2009, my maternal uncle financially helped me start a business. I started my own business in Koppal, Karnataka, and continued in Gangavati, Karnataka, from 2011 to 2015. After that, in 2015, I started a garment manufacturing business in Ahmedabad, Gujarat. This business helped me fulfill every dream of my life. In 2015, I bought a car, and my dream of owning a house in Ahmedabad was fulfilled in 2019. By the grace of God, I now own two flats in Ahmedabad, and I had become a successful businessman.
During the Corona period in 2020, during the lockdown, I ensured food, accommodation, and financial assistance of ₹5000 each to 28 people working with me and helped them reach their villages. After that, I came to my village Parloo, district Balotra, Rajasthan, with my family. I started the construction of a house in the village, which was fully completed in 2024. In between, I bought 27 bighas of agricultural land and started preparing for the housewarming ceremony. A month before the housewarming, my mother was diagnosed with cancer, due to which I had to face many sorrows. I was completely broken, wondering what to do for my mother and for the housewarming. My family members and people who knew me personally helped me a lot with my mother's cancer treatment, making every effort to provide the best hospital care so that my mother could become cancer-free. Then I talked to my mother about the housewarming, and she expressed her wish to proceed with the housewarming and bring in our Kuldevi (family deity). With the guidance of my mother and family members, and according to my wish, I brought my Kuldevi Mata Nagenechi Ji with full rituals and with all family members. A grand Jagran and Mahaprasadi program was organized on May 5, 2024. My house is equipped with all amenities as per my wishes.
Currently, my mother's treatment is going on at AIIMS Hospital, Jodhpur. During my mother's cancer treatment, the hospital visits and experiences I had will be helpful to many cancer patients. During this time, I came to know through social media about an 85-year-old elderly person who puts Jhada (traditional healing method) for cancer patients. I also wished to go there with my mother. The name of that village is Devmali, in Beawar district. I went to Devmali village with my mother and my dear friend Baburam Ji, and there I gathered complete information from the villagers and patients. The arrangements there and the people are very dear. I stayed there for three nights and had Darshan (holy visit). I was impressed by the system there. There is a temple of Lord Shri Devnarayan Ji and Bheruji Maharaj. The people there are truthful and completely believe in God and live their lives. The person who does Jhada is about 85 years old and performs Jhada from 7:30 am to 6:00 pm, and every day, Jhada is performed for three to four thousand cancer patients. Cancer patients from all over India are reaching there. There, I saw many cancer patients, and a inspiration arose in my mind that I too can help cancer patients for free. Through social media, on January 26, 2025, on my birthday, I pledged to provide free financial assistance to cancer patients for life. My aim is to get free treatment for cancer patients. I started a campaign on social media named "Cancer Mukt Bharat Abhiyan" (Cancer-Free India Campaign). For your information, I am telling you that I have a total of 38,000 followers on all my social media platforms combined. Making good use of social media, I started working to make India cancer-free. Under the Cancer Mukt Bharat Abhiyan campaign, a total of 26 people contacted me, and I assured them of all possible help. Then I thought of forming a national-level foundation with all the affluent people of the society and providing free financial assistance, and providing free financial assistance to all the financially poor people. For this, I have established Shri Rajeshwar Health Foundation. The main objective of this foundation is to selflessly help cancer patients. Through this foundation, we will arrange free ambulances in many villages.
My dear ones, cancer is such a disease that the death of the sufferer is extremely painful, and it completely ruins the family financially. I have seen many such examples in my life, the information of which I have with me. Treatment of this disease is not possible for every person. Due to this disease, I have lost many of my relatives. Friends, my financial condition is good, and I am a successful businessman. I have achieved all the success in business that every businessman wants, but due to this disease, I have not been able to go to Ahmedabad for business for the last 1 year. But by the immense grace of my revered deity Lord Shri Rajeshwar Bhagwan, I am completely prosperous. By the immense grace of Rajeshwar Bhagwan, my family members and relatives are all financially prosperous. The Cancer Mukt Bharat Abhiyan campaign started from Devmali, and I have dedicated my whole life to cancer patients.
I was born in such a large community, the name of my community is Anjana Patel Kalbi Samaj. Lord Shri Rajeshwar Bhagwan, the revered deity of my community, made many predictions 100 years ago, which have come true. My community has done many social service works in the country. People of my community mainly do farming and business all over India. Many organizations of my community are doing social service work all over India.
